There are a couple waves of the afflicted. Once you kill all of them in the general area, iirc 4 waves (2 from each side, alternating) of about 4-6 afflicted (mages and mesmers, couple warriors) come from the alleys. I remember waiting for a about 15 seconds or so for them to show up on the last wave. Then the quest will update and you can talk to Tosai and he will remove the degen. Seems you will ALWAYS be an enemy of the Am Fah though if what he said is true...should have taken a screen shot. Wonder if this effects quests in the future...bet it does.
